Telehealth Informed Consent

By using the CrownGenix platform and proceeding with a telehealth consultation, you acknowledge that you have read, understand, and agree to the following terms regarding the delivery of healthcare services via electronic communication. This consent applies to all telehealth interactions conducted through CrownGenix, including asynchronous medical consultations, follow-up communications, and prescription services.
1. Nature of Telehealth Services
CrownGenix provides asynchronous telehealth consultations, meaning that you submit your health information, medical history, and any relevant photographs through our secure online platform. A licensed healthcare provider then reviews your submission and makes a clinical determination at a later time. This is not a live video or real-time audio consultation.
Telehealth services involve the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location. The information you provide will be used by a licensed physician to evaluate your condition and, if clinically appropriate, prescribe medication.
2. Agreement to Receive Telehealth Services
By completing a consultation on CrownGenix, you voluntarily consent to receive healthcare services delivered via telehealth. You understand and agree that:
- Your consultation will be conducted through an asynchronous online platform where a licensed physician reviews the health information you provide.
- The provider-patient relationship is established upon the physician's review of your submitted information.
- You are requesting a telehealth consultation for the specific condition or treatment indicated during the intake process.
- You are at least 18 years of age and are legally able to provide informed consent.
- You are located in a state or jurisdiction where the prescribing physician is licensed to practice.
3. Benefits of Telehealth
Telehealth consultations offer several potential benefits, including but not limited to:
- Convenient access to licensed healthcare providers without the need for an in-person office visit.
- Reduced travel time and associated costs for patients in remote or underserved areas.
- Faster access to medical evaluation and, when appropriate, prescription medications.
- The ability to complete consultations on your own schedule from the privacy of your home.
- Secure and confidential handling of your health information in compliance with HIPAA regulations.
4. Risks and Limitations
While telehealth consultations are a convenient and effective method for certain types of healthcare, they have inherent risks and limitations that you should be aware of:
- No Physical Examination: The healthcare provider cannot perform a hands-on physical examination. The clinical assessment is based solely on the information, photographs, and medical history you provide.
- Technical Difficulties: Electronic systems and internet connectivity may occasionally fail, experience delays, or produce errors that could affect the quality of the consultation or delay your care.
- Incomplete Information: The provider's ability to diagnose and treat is limited by the information you supply. Incomplete, inaccurate, or omitted information may lead to an incorrect assessment or inappropriate treatment recommendation.
- Not Suitable for All Conditions: Telehealth may not be appropriate for all medical conditions. Some conditions require in-person evaluation, diagnostic testing, or emergency intervention that cannot be provided through an online platform.
- Security Risks: Despite our use of encryption and security protocols, there is a small risk that electronic communications could be intercepted or accessed by unauthorized parties.
5. Clinical Decision-Making
You understand and acknowledge that:
- The licensed healthcare provider has the sole and final authority over all clinical decisions, including whether to prescribe medication, what medication to prescribe, and the appropriate dosage and duration of treatment.
- A prescription is not guaranteed. The physician may determine that treatment is not clinically appropriate based on your medical history, current health status, contraindications, or other factors.
- If your consultation is denied or a prescription is not issued, you will be informed of the reason and may be advised to seek in-person medical care.
- The physician may recommend alternative treatments, additional testing, or referral to an in-person provider at their discretion.
- CrownGenix does not influence, direct, or override the independent medical judgment of the prescribing physician.
6. Patient Responsibilities
To ensure safe and effective care, you agree to the following responsibilities:
- Provide complete, accurate, and truthful information about your medical history, current medications, allergies, and health conditions.
- Disclose all prescription and over-the-counter medications, supplements, and herbal products you are currently taking.
- Inform the provider of any known allergies or adverse reactions to medications.
- Follow the prescribed treatment plan and dosage instructions as directed by the healthcare provider.
- Report any side effects, adverse reactions, or changes in your condition promptly through our platform or by contacting your healthcare provider.
- Keep your account information, including contact details and shipping address, current and accurate.
- Not share your prescribed medications with any other person.
7. Emergency Situations
Our telehealth services are designed for non-emergency medical evaluations only. The asynchronous nature of our consultations means there may be a delay between when you submit your information and when a physician reviews it. This platform should never be used as a substitute for emergency medical services.
8. Right to Withdraw Consent
You have the right to withdraw your consent to telehealth services at any time, for any reason. Withdrawing consent will not affect your right to future care or treatment, nor will it be held against you. To withdraw consent:
- You may discontinue use of the platform at any time.
- You may contact us at support@crowngenix.com to formally revoke your telehealth consent.
- Withdrawal of consent does not apply to services already rendered or prescriptions already issued based on a completed consultation.
9. Privacy and Confidentiality
All health information you submit through CrownGenix is handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws. We employ industry-standard encryption, secure data storage, and access controls to protect your Protected Health Information (PHI). 10. Prescription and Medication Fulfillment
If the healthcare provider determines that a prescription is clinically appropriate, the following applies:
- Prescriptions are sent to a licensed pharmacy for fulfillment and shipped directly to the address you provide.
- All medications dispensed are FDA-approved and sourced from licensed pharmacies.
- You are responsible for reviewing the medication information, including potential side effects, drug interactions, and usage instructions provided with your shipment.
- If you have questions about your medication, you should contact the prescribing physician or the dispensing pharmacy.
